Purpose and legal basis
Handling an enquiry about a service or voucher and taking steps at your request before entering into a contract: Article 6(1)(b) GDPR. Other correspondence: the controller’s legitimate interest under Article 6(1)(f) GDPR. Sending a form does not make a booking or subscribe you to marketing.
Data required and voluntary disclosure
Your name, email address, subject and message are required for us to respond through the form. Your phone number is optional. The tattoo planner also uses dimensions, descriptions, styles and placements, plus an optional link. Do not send health information or documents. You can contact us by phone or email instead.
Recipients and retention
Submitted enquiries are saved in the website’s protected database; the owner and administrator can access them in the dashboard. A copy is sent to the studio’s email address. Data is received by the studio and providers operating our forms, email and website, including FormSubmit and Google. We retain it for the time needed to handle your enquiry; where a contract or legal claim is involved, further legal bases and retention periods may apply. Our privacy policy explains providers, possible transfers outside the EEA and retention criteria.
Your rights
Subject to the GDPR, you have rights of access, rectification, erasure, restriction, data portability and objection. You can withdraw consent for optional embedded content. You may complain to the President of the Polish Personal Data Protection Office (UODO). We do not use our forms for automated decision-making or profiling.
